This form is a request to enroll my child at Team Success for the 2024–2025 school year
Completing this form does NOT guarantee a spot in the desired grade level
I will be notified about the status of the request once the lottery has been completed
Forms submitted after February 28, 2025 will be considered for later lottery processes
Siblings of current students will be considered first for any available spots
